With the ongoing concerns of COVID-19, we are trying our best to keep our community safe. We are asking everyone to agree to the below health + conduct declaration:

• Tested positive or presumptively positive with COVID-19 or been identified as a potential carrier of COVID-19 virus
• Experienced any symptoms commonly associated with COVID-19
• Been in direct contact with or the immediate vicinity of any person I knew and/or now know to be carrying COVID-19 or has travelled in the last fourteen (14) days
